The Family Nurse Practitioner reports to the CEO/Clinic Medical Director. This position plays a key role in providing holistic medical care to a wide range of clients. The ideal candidate for this role embodies the essence of “caring for others like their own family members” and creates a welcoming environment for clients seeking the highest quality care in their local community. From performing comprehensive histories and physical examinations, conducting well and sick visits, administering immunizations to adhering to evidence based clinical practices, the FNP helps guide clients to achieve better health.

DUTIES AND RESPONSIBILITES TO INCLUDE:

1. Assist Medical Director in developing clinical practice guidelines.

2. Establish positive relationships with local medical community, attend local medical society functions and other community health events.

3. Interview patients to obtain history, provide health maintenance visits, evaluate for immunizations and provide anticipatory guidance and referrals for secondary and tertiary care.

4. Perform contract and special physical examinations as: EPSDT, Head Start, employment, etc.; performs STI (sexually transmitted infection) and family planning screenings and education; perform physical examinations, orders lab and other tests, blood draws, prescribes medications and treatments.

5. Provide continuity in managed care for patients with pre-existing long-term conditions.

6. Document and code all patient contact accurately in the electronic health record.

7. Performs as an active member of the medical team and an active participant in provider and medical care team meetings.

8. Participates in all value based programs and may be asked to perform other duties that align with our mission, including but not limited to participation in quality review, providing education and/or other activities.

EDUCATION AND/OR EXPERIENCE:

• Graduate of an accredited Nurse Practitioner Program AND certified by the American Nurses Credentialing Center or the American Academy of Nurse Practitioners.

• Be licensed as a nurse practitioner to practice without restrictions within the State of Florida.

LICENSE AND/OR CERTIFICATION:

• Current licensure as a NP to practice in the state of Florida

• Current DEA and CPR certification Company Description
